Add "how did you find us" to customer checkout

I am looking for a way to either add an "how did you find us" text field to the customer registration page or somewhere in the checkout area but I can't figure out how. Is there an script I can use to make this, or a cheap module somewhere that can help me with this?

Look in the Addendum Additional Checkout Questions category in my module directory. I would go with Addendum from Weiland or Additional Checkout Data Pro from Sebenza. If you need v5, look for the modules with - v5 in the title. hth
