CFMAIL and Attachments

I am trying to setup an HTML form through CFMAIL that allows a user to
send an attachment once the have filled out the form. Does anyone know how to do this?

Have you looked at this tutorial? It's for sending multiple attachments, but I'm sure you can trim it down to accomplish what you need.
